Modelling Success: A Case for Consumer Directed Care

The Program

Three high-rise condominium buildings, located at 96 Street and 79 Avenue, just off Mill Creek Ravine, provide the beautiful setting for this one-of-a-kind Home Support Program.

Accessible features built-in to many of the suites allow adults living with various physical disabilities, to maximize the opportunities that life provides.
An on-site support service, staffed by Health Care Aides, an LPN, and managed by a Coordinator and Board of Directors, provides the necessary day-to-day services that help to enable individuals to live as independently as they can within their own homes.
“To me independence is all about being able to get out into the real world and to go to school, get a job and not have things slowed down by my disability. Creekside support services have (had) allowed me to be able to get out to the university and they have (had) helped me at school… Creekside is allowing me to achieve my goals.”
